Request for Administration of Medication by School Personnel

In the event your child needs to take prescription medication while at school, the preschool Request for Administration of Medication by School Personnel form must be completed by the child’s physician and parents and returned to the preschool office.

Any changes to a student's home address, contact numbers, emergency contact numbers, custody, etc must be updated on the Change of Student Information Form.
